Better to insist on physician review of immunization status at defined ages (2, 4, 6, 18, 50, etc.) "Did you review the record?" and "Are the immunizations up to date?" This would make the physician responsible for ensuring that all patients in his practice receive age specific immunizations from some source and that these are recorded in his records.
